Family up to 4 $200.00 (ea. additional child $25.00; children to age 26)
DOT Physicals $75.00
One-time acute visit for non-members: $150.00 (additional costs for labs/treatments - see price list below). These are acute sick visits only. No maintenance prescriptions or treatments will be provided. Maintenance and management of prescriptions or treatments are reserved for members of our practice.
